Community Care aims to provide care, services and support to enable people affected by problems resulting from ageing, learning, physical or sensory disability or health problems, to live as normal a life as possible in their own homes or in their local community.

Access to Community Care services is via an assessment of the individual’s care needs by a Care Officer. Carers can also request an assessment of their needs.

Services can be arranged to meet an individual’s needs; these will be reviewed on a regular basis to ensure that they continue to meet the client's needs.
