The team is involved specifically with all looked after and accommodated young people aged between 14 - 19 (or 21), and provides support both practical and emotional to enable them to make the transition from being looked after to living independently in the community.  Referrals come from social workers in the area teams when the young person is aged 14 years.

• Scottish Throughcare and Aftercare Forum - is a voluntary organisation whose aim is to improve support for young people leaving residential or foster care in Scotland.
