1. Does the local authority hold data for the number of people waiting for a social care assessment or receiving a social care package with the primary need of Motor Neuron Disease (MND)?

a) How many people with a primary need of MND are currently waiting for a social care assessment?
b) How many people with a primary need of MND who have been assessed are currently waiting for a social care package?
c) How many people with a primary need of MND are currently receiving a social care package?
d) If no, how many people are currently waiting for a social care assessment who have a terminal illness diagnosis?
e) If no, how many people who have been assessed are currently waiting for a social care package who have a terminal illness diagnosis?
f) If no, how many people are currently receiving a social care package who have a terminal illness diagnosis?

3. For the year 2025/2026, what was the average length of time between receiving a social care assessment and a social care package being implemented? Please break this down, where possible, by:

a) all those aged 18 and above in receipt of a social care package
b) those aged 18 and above with a terminal illness in receipt of a social care package
c) those aged 18 and above with a primary need of MND in receipt of a social care package, if available

4. Does the local authority have a prioritisation system for social care assessments and packages? If yes:

a) Can you provide a link or document outlining how individuals are prioritised?
b) Do those with a rapidly progressing terminal illness have access to expedited assessments and packages?
c) If yes what is the average time frame from requesting a package of care to this being implemented for those with a terminal illness?

5. Does the local authority hold data on the number of unpaid carers they currently support who care for someone with a terminal illness, and, if yes, what are those numbers?
