1. The case management system used to record the work of your Mental Health Officer service: the system name, supplier, contract start date, contract end date and any extension options, and any recorded plan to replace or re-procure it before 31 March 2029.

2. How Mental Health Officer reports and social circumstances reports under the Mental Health (Care and Treatment) (Scotland) Act 2003 are currently completed and held: on paper, as word-processed templates saved to a file store, or within the case management system.

3. How your Mental Health Officer service currently sends statutory forms and reports to health boards, the Mental Welfare Commission and the Mental Health Tribunal for Scotland, for example by secure email, post or an online portal.
