Who We Are
Young people aged 14-18 years can be referred (usually by their school) to the Moray Health & Social Care Partnership Social Work Transitions Service for an assessment of their needs for future support as an adult. We will also assess if they meet the partnership's Eligibility Criteria.
If they do, a Social Worker for Transitions will support the young person who is likely to require support as an adult due to a learning disability, mental health issue or physical disability. We work in partnership with other agencies such as education, NHS Grampian, Grampian Housing, voluntary and private agencies - as well as parents or carers - to ensure that young people in Moray remain supported.
What Our Service Can Provide
A Social Worker for Transitions receives information from school, and will:
- Link with schools, Children’s Services (if involved) and other agencies
- Attend relevant meetings, including those arranged by schools, Social Services or other professionals
- Undertake an individual assessment of need and risk, for future support
- Identify and recommend an action plan
- Signpost to other services
- Undertake short term work
- Link the young person with specialist services, as required
- Maintain contact with the young person, his/her family and relevant others to ensure communication throughout the Transitions period
Transitions Panel
A multi-agency Transitions Panel exists where there are complex issues or funding implications.

We record all details given in order to ensure the young person receives the best service possible, in line with our legal requirements.
What You Can Expect From Us
When a young person has been assessed as requiring support from Transitions, you have:
An allocated worker: You will have an allocated Social Worker for Transitions.
Professionalism: We work to the Code of Practice for the Scottish Social Services Council; and Moray Health & Social Care Partnership's procedures.
An understanding of our Eligibility Criteria: Services are provided where individuals meet our Eligibility Criteria for Adult Services.
An Individual Needs Assessment: A social worker will undertake an assessment of need.
Financial Assessments: We operate a fair, legal and equitable Contributions Policy.
Carers’ Assessments: We support unpaid carers by undertaking an assessment of their needs.
Agreed Care Plans: We agree and review care plans regularly with your involvement.
Confidentiality: Our work remains confidential within services. We have a duty to share any concerns in order to keep young adults safe.
Advocacy: We involve young people at all times and can link you to an independent advocate.
