FOI Request - Active Councillors By Year

Under the Freedom of Information Act 2000, I would like to request the following information:
1. A yearly list of all councillors, from shortly after the May elections in each year, who have been elected on the council from the year 2000 to 2016.
2. For each councillor, please include their name, party, and ward, and where possible their start and end dates of service.
