I would be grateful if you could provide the following information in relation to Self-Directed Support (SDS) across Moray for the last 12 months:

1. Number of assessments: How many SDS assessments have been undertaken across Moray during this period, including assessments completed by social work teams and those undertaken in relation to unpaid carers?

2. Allocation of SDS options: Of the assessments completed, what proportion of individuals were allocated to each of the four SDS options?

3. Implementation of SDS options: Of those allocated to each option, how many have subsequently had their chosen option implemented?
