Regarding social work within hospital settings in Scotland, I am looking for information in relation to the following: -
1. How many staff are aligned to your hospital social work team (specifically looking at discharge where social work intervention is required and not inclusive of referrals to care at home only);
2. What is the composition of this team in terms of Team Managers/ Team Leaders/ Mental Health Officers/ Social Workers/ Social Work Assistants/ Business Support or Admin/ Any Other Staff;
3. How many staff are aligned to your hospital care at home discharge team;
4. What is the composition of this service similar to question 2;
5. How many hospitals does your hospital social work team cover;
6. Where is your hospital social work team based primarily - does it differ to the care at home service looking at hospital discharge;
7. On average - how many referrals does your hospital social work team receive on an annual basis (split between social work intervention and care at home only);
8. What is the overall population of your authority area; and
9. What % are over 65.

1. 7 staff, 2 of whom are part time.
2. The team is composed of eight members of staff; the roles include Team Manager, Advanced Practitioner, Social Worker, Social Work Assistant and Admin.

3. None - Our care at home service do not have a dedicated hospital discharge team.

5. We cover Dr Gray’s Hospital, 3 Community hospitals in Moray and any hospital in Scotland that a Moray resident is referred from
6. Flexible working but based in Dr Gray’s Hospital, SWs also work out of the 3 Community hospitals and work at home
7. There was 542 referrals last year.
