1.  Waiting Times for Each Stage of the Respite Care Process

For each calendar year over the past five years, please provide the shortest, longest, and median waiting times for:

•  Initial contact made to the Local Authority requesting respite care support to the start of respite care.
•  Initial contact made to the Local Authority requesting respite care support to assessment
•  Assessment to decision/approval
•  Decision/approval to allocation of respite care
•  Allocation to actual start of respite care

Please include the same breakdown for cases where respite care is accessed via Self-Directed Support (SDS) Direct Payments, covering:

•  Initial contact made to the Local Authority requesting respite care support to issuing of funds for respite care through SDS Direct Payment.
•  Initial contact made to the Local Authority requesting respite care support to assessment for SDS.
•  Assessment to approval of SDS Direct Payments
•  Approval to payment setup and funds available for respite care

2.  Breakdown by Type of Respite Care

Please include this information for all forms of respite care offered by your authority, broken down by type (e.g., residential respite, in-home respite, day services, etc.), and indicate where SDS Direct Payments were used.

3.  Current Waiting List

Please provide the number of people currently waiting for respite care, broken down by:
