1. Does your local authority have a policy in place to provide education to pupils of school age if they are hospital inpatients? (if yes please provide a copy of the policy)

2. Does your local authority provide in-person education to pupils if they are resident or inpatients in
hospital or other medical facility?

3. Does your local authority provide remote education to pupils if they are resident in or an inpatient
hospital or other medical facility.

4. If a pupil from one of your schools is resident or an inpatient in a hospital outside of your local
authority area does the local authority provide the education to the pupil itself? (If so please provide
details of how this is delivered)

a) If not, what is the procedure for providing education to a pupil who is resident in another local
authority due to illness?

b) If your Local Authority has an arrangement with another local authority to provide hospital education
to your pupils could you please share the agreement or outline the nature of the arrangement.
