Please can you send me a copy of the Criminal Conviction Declaration Form given to employees to complete for interviews that require a higher level disclosure.

The criminal conviction declaration is part of the standard application questions on our application form  and reads "Do you have any unspent criminal convictions or impending charges against you?  (You do not need to declare any "spent" convictions). 

This question is relevant to applicants applying for any post, not necessarily a post whereby an applicant needs to be PVG/disclosure checked.  Should the post they are applying for require a PVG or Disclosure check this would be carried out prior to successful candidate commencing in post.
